An Elder Care Manager is educated and trained to assist older adults and their families with challenges such as housing, home care services, medical management and legal and financial issues. An Elder Care Manager acts as a liaison between the client, family and other professionals while providing a holistic client-centered approach to care coordination.

Does my insurance cover these services?
Medical Insurance will often pay for short term interventions through Home Health Agencies. These agencies provide skilled nursing, physical therapy, occupational therapy, social services, speech therapy and home health aides. Care management services are not currently covered by medical insurance, however, if you have a long term care policy, check your policy directly as some policies have coverage.
Are there free Services?
Yes, there are government and community-based programs that provide care management, however, these can be limited, fragmented and uncoordinated and many clients fall through the cracks. Investing in an independent Care Manager ensures that your loved one has personalized care coordination during every interaction.
